Long-Term Care Initiative

The Family Practice Services Committee (FPSC) long-term care initiative supports Divisions of Family Practice to design and implement local solutions that deliver dedicated Physician Most Responsible Practitioner (MRP) services for patients in long-term care homes.  Given the interconnected nature of the health care system, improvements and efficiencies will be maximized by the local division working in close collaboration with Interior Health and participating long-term care homes.  The LTCI in the Central Okanagan was launched in July 2015.

For the purposes of this initiative, a Physician MRP is defined as one who promotes the three system level outcomes and who delivers care according to these five best practice expectations (BPEs):

​

1) 24/7 availability and on-site attendance, when required
2) Proactive visits to residents
3) Meaningful medication reviews
4) Completed documentation
5) Attendance at case conferences

 

System level outcomes:
1) Reduced unnecessary or inappropriate hospital transfers
2) Improved patient-provider experience
3) Reduced cost/patient as a result of a higher quality of care

Primary Care Network
  • Primary Care Networks (PCNs) are a collaboration between the Central Okanagan Division of Family Practice, Interior Health, and local Aboriginal partners to provide comprehensive primary care services that coordinate with specialized care and community-based services in the Central Okanagan.

  • Priorities of the PCN are to provide team-based care through an interdisciplinary team of nursing and allied health professionals, to increase access for the following priority populations: frail in community; low and medium complex conditions and; those with mild to moderate mental health and substance use.

Physician/Locum Recruitment & Retention
  • Our physician recruitment and retention program has been ongoing for many years and now includes a centralized application process on our website where family physicians can view all the currently available positions. When a physician/locum applies for a position at your clinic, our Division will send you their details.

MOA Recruitment and Retention
  • Our MOA and Office Manager Recruitment service, initiated in late 2021, assists physician members by managing the hiring process. This includes posting job listings, reviewing resumes, conducting screening interviews, and providing access to screening notes, offer letters, and HR onboarding materials – all at no cost to our members. The Division is here to simplify the recruitment process, recognizing the time-consuming and challenging nature of hiring office support staff.

MOA Network
  • We recognize the important role of MOAs in supporting primary care and have launched our MOA Network for the purposes of engagement, education, and support to Medical Office Assistants and Office Managers employed in our members' clinics. This initiative brings together MOAs from our community with an emphasis on on education, upskilling and networking.

Division Events
  • Continuing Medical Education (CME) events and member socials are held approximately 6 times per year via Zoom or in-person. In-person events allow for physician networking over dinner before guest speakers present.

Patient Attachment: Health Connect Registry
  • Our Division is working with the provincial program called the Health Connect Registry to attach patients with a family physician in their local area. Patients can go to the HealthLink BC website or phone 8-1-1 to join the waitlist for a practitioner. When one becomes available patients will be contacted.

  • Physicians who have space for new patients can contact the Division at members@codivision.ca to let us know how many patients they wish to accept.
