PCN For Patients
A Primary Care Network (PCN) is a collaborative effort between family physicians, nurse practitioners, clinicians, and allied healthcare providers dedicated to improving patient health care experiences. Created by the BC Ministry of Health, our PCN is a partnership between the Central Okanagan Division of Family Practice, Interior Health, and Indigenous partner organizations. Our mission is to empower individuals of all ages with essential healthcare information, ensuring a seamless collaboration between care providers for a comprehensive primary healthcare experience.
PCN For Physicians
Family doctors who participate in the PCN/PMH initiative benefit from enhanced support, expanded services for vulnerable populations, and the opportunity to shape and influence local PCNs. Patient Medical Homes (PMHs) serve as the foundation for PCNs, offering continuous, comprehensive, and coordinated care, and allowing family doctors to focus on quality patient care and prevent burnout.
